Her professional background includes over 25 years in acute and chronic disease management and psychiatry as a registered nurse. She lives in the Live Oak area with her family.

Karen E. Valentine

Karen currently follows our patients at HarborChase of Gainesville ALF, Hunter's Crossing ALF, and provides weight loss services at Elite Medi-Spa. As a Florida native, Karen grew up in Ormond Beach, Florida. She is a wife and a proud mother of 5 children. She is an active member of her church and a lover of animals as in her spare time she also is a well known and successful dog breeder. Karen grew up in a large family and was raised by a prominent family physician, who was her father, and a brilliant business woman for a mother who was her mentor and ran her families several owned medical clinics. They spent many years training her in various roles throughout their many clinics. She fell in love with serving others and began her formal journey into the medical community. Karen graduated with her ASN from Daytona State College and obtained her BSN and MSN from Jacksonville University. She has been a nurse for 18 years. She initially was trained as a trauma neurosurgical nurse at a level 2 trauma center. Karen moved to Gainesville in 2010 and since obtaining her APRN certifications she has worked primarily in geriatric medicine in acute rehab facilities, long term care, assisted living facilities, and HCA North Florida Regional. She also provides services in Bariatric medicine as this was her initial interest and basis of her thesis for her MSN. She also works in palliative/hospice care. She additionally has been trained as a neurosurgical APRN in which she proudly worked with a local prominent neurosurgeon for several years.

Bonnie Bosier

Bonnie will be covering Alachua, High Springs, Newberry, Fort White, Lacrosse, and Gainesville areas. She is currently accepting new patients in those areas for in-home primary care. Bonnie is an AANP board-certified Family Nurse Practitioner. She received her Bachelor of Science in Nursing from the University of Florida in 2001. She later earned her Master of Science in Nursing from the University of St. Augustine. Her professional background includes working over 20 years in the Emergency and Interventional Radiology Departments as a registered nurse. She is passionate about nursing and is excited to provide care for people in their homes. In her spare time, she enjoys anything outside or near the water. She lives in the Alachua area with her husband and four children.

Kendra Claude Fugate

Kendra Fugate is a doctorate-prepared board-certified family nurse practitioner. She has worked as a nurse practitioner since 2021 in both primary care and cardiology settings. Prior to working as a nurse practitioner, she was a registered nurse in a pediatric cardiac intensive care unit for three years. Kendra earned a bachelor’s of science from the University of Florida in 2014 and then earned her bachelors of nursing from Sante Fe College in 2017. She then earned her Doctor of Nursing Practice from the University of Florida in 2021. Kendra is married with two children and enjoys spending quality time with her family. She was raised in Williston Florida and feels passionate about providing primary care to her local community. Her goal is to provide personalized primary care forming a collaboration with her patients. She wants to ensure her patients know they are more than their medications and they all deserve a personalized care experience.

Corinne Riley

Corinne joins us to continue to expand to those in need of mental health services in the Gilchrist, Levy, Dixie and Alachua county areas. Corinne is an ANCC Board Certified Psychiatric Mental Health Nurse Practitioner. She holds a Bachelor of Arts in Psychology and Sociology and a Bachelor of Science in Nursing from The University of Tennessee. She recently obtained a Master of Science in Nursing degree from Walden University. Corinne has seven years of experience in mental health as a registered nurse. She possesses a vast amount of knowledge in the field of psychiatry as her experience ranges across the lifespan, with her most recent experience dealing with crisis treatment and stabilization on inpatient psychiatric units. Corinne returned to Florida in 2017. She currently resides in the tri-county area with her husband and three children. Her hobbies include being outdoors, staying active, mudding, traveling, and spending time with those that mean the most. Corinne takes pride in providing care to those in need and looks forward to making a change as she serves our community.

Where does the story begin? I moved to Florida in 1984 after meeting my future husband in East Tennessee where I was born and raised. I started out nursing as an LPN in a small rural hospital working in ICU/ED and over the next 6 years obtained my RN. After a short time in nursing, I worked my first shift in ED and fell in love with Emergency Medicine. I was an ED nurse and a Flight nurse for many years with more stories and experiences that could fill a book. The challenges I confronted could be overwhelming but to see someone critically injured walk back into the department so say, “Thank You”, was worth it all. I finished my BSN and later enrolled in Master of Nursing for Family Nurse Practitioner. I have held many roles over the years as a staff nurse in ED, ICU, Cath Lab, Flight Nurse to name a few and then began working as a Clinical Nurse Educator in several different Emergency Departments in North Central Florida. I started teaching classes and precepting new nurses and another passion was ignited. I continue to teach trauma and pediatric classes for the Emergency Nurses Association and precept nurses in their clinical sites to become Nurse Practitioners. I have worked in rural healthcare as a Family Nurse Practitioner since 2006. I am board certified Nurse Practitioner in Family Medicine, Certified Emergency Nurse, Certified Pediatric Emergency Nurse, and Trauma Certified Registered Nurse. I am an active member of the Emergency Nurses Association, and I am the Florida Immediate Past State President and serve on two national ENA committees. My husband and I have been married for 40 years and have one son, 2 West Highland Terriers or terrorists as we call them, an older horse, my Koi fish, and feed the wild deer and turkey on our property. My hobbies are gardening and digging in the dirt. My other hobbies are being a motorsports fan of NHRA Drag Racing and Formula 1. I am a diehard Tennessee Volunteer fan of any sport even though I attended the University of Florida. Many have tried to make me a Gator fan and have not succeeded. I like to travel and experience new places and new cultures. I enjoy working with patients of all ages and getting to know them. I try to be honest and open to patients’ concerns or issues and work with them to develop a care plan to help them meet their medical needs.

Meet the wonderful Mrs. Madeleine Kelley! A wonderful addition to our amazing group of Nurse Practitioners providing home-based psychiatric & mental health services to North Central Florida! Maddie is an ANCC Board Certified Psychiatric Mental Health Nurse Practitioner. She holds a Bachelors of Science degree in Sociology from Jacksonville University and a Bachelors of Science degree in Nursing from Chamberlain University. She is a recent graduate of Maryville University where she obtained her Masters of Science in Nursing. She is currently enrolled in the Doctorate of Nursing Practice program through Chamberlain University. Maddie has practiced as a registered nurse for the past seven years, primarily in psychiatric and mental health. She also has experience working in the home health field. She currently lives in the Lake City area with her wife and dog children. When she's not hitting the books or seeing patients, Maddie likes to experiment in the kitchen, travel, work out, and participate in community events. Maddie is excited to contribute to the mental health crisis solution in our community, state, and nation!

Meet Bobbie-Jo Burke CNA, Case Manager. Bobbie-Jo has been in the medical field for 16 years and is experienced in many different roles. She takes pride in working hard and learning new things. She has experience ranging from working as a CNA, Home health aide, Referrals Processor, Nursing Home Admissions, Marketing, Clerical, Triage, Authorizations, Staffing Coordinator, Covid Coordinator, Human Resources, and now currently hold a spot here as part time Case Manager. She lives with her husband and their two boys. She likes to stay busy and is always on the go with her kids traveling for baseball and competitive bass fishing. Though it is a lot, she states that she wouldn't have it any other way.

Please meet Mrs. Tammy Snedeker, Case Manager Supervisor with Andrews Healthcare Consulting. Tammy has been working with Andrews Healthcare Consulting since August of 2022. She born in Gainesville and is a Florida native, where she is a Dixie County High School graduate. She currently resides in Old Town, with her husband and daughter. She enjoys spending time with her family and friends and watching her daughter do her many activities, with one of her favorites watching her play softball. Tammy has been in the healthcare industry for over 10 years in many different roles including physician's offices and private practice. She enjoys being able to interact with all the patients as well as being able to help with any questions or concerns they may have.

Hello, my name is Raigan NesSmith. I am from Cross City, FL and I am 23 years old. I went to DCHS and graduated with the class of 2018, then I graduated from FGC in 2022 with my AA degree. While working with Andrews Healthcare, I have had quite a few roles. When I first started, I was answering all the phone calls and returning voicemails, sending faxes, submitting orders and various other things. I then went on maternity leave to spend time with my new baby boy. I am back working with the providers to get their orders done and help them in any way I can, as well as handling the weight loss patients along with Kaylee. In my free time I enjoy being on the water, fishing, hunting and spending time with my family.

Logan Andrews

Logan Andrews, MSN, APRN, FNP-BC is a licensed autonomous Family Nurse Practitioner. Born and raised in the Tri-County area and a graduate of Chiefland High School, Logan knows how difficult it can be to find access to affordable and reliable primary care services in rural North Central Florida. Logan has been in the healthcare industry since 2007 in various roles and positions.  Logan received his Associate in Science in Nursing degree from Florida Gateway College and his Bachelor of Science in Nursing degree from Jacksonville University. After working many years in post-acute care in the skilled nursing and long-term care setting, he obtained his Master of Science in Nursing degree from Purdue University Global. He has worked as a nurse practitioner in multiple area skilled nursing facilities, ALFs, and at both North Florida Regional Medical Center & Select Specialty Hospital.

Jennifer “Jen” Cassisi is an ANCC board certified, autonomous practice, Family Nurse Practitioner. She received an Associate of Science in Nursing degree in 2001 from Broward College in Ft. Lauderdale, then continued on to receive her Bachelor of Science in Nursing degree from the University of Florida in 2007. She worked for several years as a registered nurse in the operating room and the surgical intensive care unit allowing her to gain invaluable knowledge that she continues to draw upon in her role as Family Nurse Practitioner. After several years working as a registered nurse, she completed her Master of Science in Nursing degree from the University of Florida. Since becoming board certified, she has worked as a nurse practitioner in office-based primary care settings since 2011. Jen is a proud mother to 2 teenage children and a sweet 6-year-old Goldendoodle. She is an avid tennis player, as well as a travel and exercise enthusiast. She is a native Floridian who relocated to Gainesville from Ft. Lauderdale over 20 years ago, but now considers Gainesville and the surrounding areas her home. She will be providing Primary Care services in the to the Eastside of the Gainesville area.

Her professional background includes many years in emergency medicine and sub-acute care as a registered nurse. She lives in the Mayo area with her family.
